I've recorded a very serious and thorough medical exam questionnaire in French, with English translation and French audio.
The questions and answers of this physical exam in French were developped with the help of 2 US doctors and an anesthesiologist nurse. It's been used by many doctors since then, so it's reliable.
I don't know that anything else like that is available on the web, so please, pass the info around to any person, organization, etc... who could be interested.
